VALE residents will have the opportunity to find out about the hospital discharge service at a special meeting.

The Vale Co-Creating Healthy Change Forum will meet at Llantonian Hall, Boverton Road, Llantwit Major, on Wednesday, December 3.

The free event, organised by the Co-Creating Healthy Change Project at Diverse Cymru, will be held at Llantonian Hall, Boverton Road, Llantwit Major from 2pm to 4.30pm.

Shelagh Maher, of Diverse Cymru, said: “The Hospital Discharge Service provides short-term intensive support to older people in their own homes following discharge from hospital. We hope that people will come along to find out more about this service and put forward any questions/views they may have.

“Everyone is welcome to attend the forum, particularly Black and Minority Ethnic people, carers, disabled people, lesbian, gay and bisexual people, older people and transgender people, as people from these backgrounds don’t always have their views heard on the services they receive.”

No special knowledge or experience is needed, refreshments will be provided and expenses will be offered for travel, access and replacement care costs - including child care.
